Special Needs: Completed HW Treatment but still ligth positive - need DFIL test at RBAH in 6 months - if at that time she re-tests postive ASC will re-treat at our cost - test must be done at RBAH. Must be kept on Revolution Rabies shot can be given at Wellness visit if cert can not be issued based on form
